Our Hospice at Home team: providing care and support when it's most needed

This Hospice Care Week we salute the incredible work of our Hospice at Home team who provide hands on nursing care and support for patients during their last days of life, enabling them to remain at home.

It may surprise you to know that 90% of Hospice care happens at home through a combination of services including our Hospice at Home team, Community Nurse Specialists, Medics, Patient and Family Support and our therapists.

Hospice at Home Health Care Assistants

Hospice at Home Senior Healthcare Assistant, Mel says: "We provide personal care and help to support our patients' families and loved ones, and those they care about. Caring for a loved one is a full-time job and can be very lonely at times.

"We also offer respite visits which means that the person caring for the patient can have a break. We offer night shifts on an ad-hoc basis, and this can help them to get some rest. We're there to give support and to help alleviate pressure, practically and emotionally.

"Our visits are not set to times, so we can offer as much flexibility to our patients and their families. It's a privilege to look after people in their own home at this time in their lives. I've had family say to me that you're the angels that we didn't know about."
