Supporting personalised care for people approaching the end of their life involves finding opportune moments to sensitively talk about what their priorities are, and how to enablesuch conversations to inform decisions relating to their end of life care.

Deciding upon the right way to approach end of life converstaions is ofte  difficult. However, enabling people to talk about and plan ahead can influence the quality of end of life care offered to individuals and their families.

This one-day course is suitable for any registered practitioner (Nurse, AHP, Dr) working with adult patients with long-term conditions/life limiting illness (COPD, Heart Failure, Stroke, Neurological Conditions, Elderly Frail) within South Gloucestershire, Bristol and North Somerset.

  • How to sensitively approach end of life conversations
  • Helpful approaches to supporting person lead end of life care
  • Advance Care Planning processes, their legality and application in practice
