All entrants must agree to the following terms and conditions:

Entrants must be 16 years of age or over on the night of the event, Saturday 7 July 2012. An adult guardian must accompany each entrant aged 16 – 17 years for the duration of the Midnight Walk. The guardian must sign this form in the space provided and separately enter in The Midnight Walk. The applications must be submitted together. The full entry fee is applicable.

Entries will close on Friday 29 June 2012 (subject to the number of entries received prior to this date).

Once accepted, your place cannot be transferred to any other person. If you cannot attend the walk, you must inform St Peter’s Hospice as soon as possible.

Personal details: all entrants must provide a home address with postcode, together with an emergency contact name and telephone number of someone who will be available on the night of the event. This should not be someone participating in the walk.

You must not enter on behalf of someone else. By filling out this form you agree that all the information that you have inserted is correct.

The registration fee is non-refundable. In exceptional circumstances refunds may be given at the discretion of the management.

Sponsorship forms will be sent to you with your confirmation letter.

No animals apart from guide dogs are permitted.

No alcohol to be brought to the event site or consumed during The Midnight Walk.

Disabled entrants are welcome to take part but we ask that you contact us prior to completing the form as some people may have difficulties with the route. If a disabled entrant requires assistance from another person, this person must also enter as an individual. Only guide dogs are accepted – no other pets or animals are permitted.

The Midnight Walk is not a race. No walkers are allowed to run or jog and no walker must pass the nominated lead walkers.

Medical Information - You are responsible for your own medical condition and all participants walk at their own risk. St Peter’s Hospice will not accept liability for any injury or loss as a result of your participation but your statutory rights remain unaffected. You agree to seek medical advice from your general practitioner if you are in any doubt as to your physical ability to participate in the event.

By saving the entry form you agree to follow the directions of St Peter’s Hospice and abide by the rules and conditions of entry for The Midnight Walk.

Due to unforeseen circumstances, if necessary St Peter’s Hospice reserves the right to cancel this event at any time.

By registering you are giving your permission for any photographs taken of you on the night to be used in future publicity.

I confirm I have read and accepted St Peter’s Hospice terms and conditions. I wish to enter The Midnight Walk and understand that I do so at my own risk and that St Peter’s Hospice will not accept liability for any injury or loss as a result of my participation but I understand that my statutory rights remain unaffected. I agree that I should seek medical advice from my general practitioner if I am in any doubt as to my physical ability to participate in the event.
