Identifying End of Life and Planning Ahead

Recognising when patients are coming to the end of their life is often difficult.  Knowing how to support those individuals and their families with Advance Care Planning is then something that many practitioners don’t feel confident about.

This one-day course is suitable for any registered Health and Social Care practitioner (Nurse, AHP, Dr) working with adult patients with long-term conditions/life limiting illness (COPD, Heart Failure, Stroke, Neurological Conditions, Elderly Frail) within South Gloucestershire, Bristol and North Somerset.

The programme includes

  • The tools and processes that can help practitioners identify when individuals are coming towards the end of their life

  • Helpful approaches to planning end of life care in the last year of life

  • Role and processes that support Advance Care Planning including their legal status
