Long Term Conditions

Many people live with what is often referred to as Long Term Conditions (COPD, Heart Failure, Neurological Disease, Stroke etc.) for many years.  They are often well supported by a whole range of professionals who provide care during acute episodes of illness and during periods of stability and rehabilitation.   However, for many individuals there comes a time when their condition deteriorates and they enter a phase which is called ‘End of Life.’

One of the significant challenges for practitioners caring for these individuals and their families is identifying when end of life care is appropriate, and how to confidently respond to their holistic needs including planning ahead.

These two study days are aimed at enabling practitioners to develop their knowledge, skills and confidence in caring for individuals with long term conditions at end of life.
